Summary: A4 printout misaligned on paper

Initial Comment:
When printing A4 sheet on A4 paper (Orientation: Landscape, Type: Extent with margins), the printout is shifted right by about 3cm (1.2"). Rightmost part of the sheet is beyond printing area and remaining margins are: top 10mm, bottom 9mm, left 40mm.
Similar effects can be seen if the sheet is printed to file and loaded into gimp or viewed with KPDF.
